Acton Public Schools Student Registration
Welcome! Once you are settled in the Acton area, please contact Ruth Cvitkovich, our Registrar at 978-264-4700 x3214 to schedule an appointment to begin the registration process. Please bring copies of the following student document with you to your appointment:
- Copy of birth certificate OR passport
- Proof of Residence (Purchase & sales agreement, copy of lease or electric bill)
- If applicable, any current Individual Education Plan (IEP for Special Education students).

Please download and complete these four forms prior to your appointment:
In addition, your child's medical records are required at enrollment. These records maybe obtained from your family doctor. Please bring the following with you:
- School Health Record Form (a copy of last physical examination)
- Certification of Immunizations
These additional forms are used for Kindergarten Registration. Please have your Early Childhood program complete this Pre-kindergarten Assessment Form.
